Oak Lawn Respiratory & Rehabilitation Center - POSITION
SUMMARY:We are seeking a passionate Registered Nurse (RN) Or
Licensed Practical Nurse (LPN) to join our team as a Wound Care
Coordinator. - The Wound Care Coordinator reports to and receives
general direction from the Director of Nursing and is responsible
for delivering quality wound care and preventative measures to
maintain skin integrity to assigned residents in compliance with
applicable federal and state guidelines and established policies
and procedures.
ESSENTIAL JOB FUNCTIONS:--- Utilize the nursing process by
demonstrating appropriate assessment, planning, implementation and
evaluation of residents wound and skin integrity--- Collect and
document residents' skin status including medical history.--- Wound
care prevention and prevention through plan of care--- Set
priorities for nursing action for wound care and prevention---
Communicate with physicians regarding resident wound needs---
Initiate nutritional interventions to ensure that residents with
wounds are provided nutrition to promote wound healing.--- Perform
direct resident care utilizing established procedure, policies, and
standards--- Administer treatments and procedures in accordance
with the physician's orders--- Maintain the comfort, privacy, and
dignity of each residents while performing treatments---
Communicate and interact effectively, courteously, and tactfully
with residents, visitors,family members, peers, and supervisors---
Conduct rounds frequently to ensure that residents with
incontinence are receiving pericareand preventative ointments
timely. Discipline non-compliant staff accordingly withconsultation
from the DON--- Perform duties in a safe manner in accordance with
established policies and procedures--- Perform all job
responsibilities in accordance with prescribed safety and infection
controlprocedures including thorough hand washing, use of
disposable gloves and proper disposal ofsoiled material
EDUCATION/EXPERIENCE:
- Graduate of an accredited school of Nursing- Must possess a
current and unencumbered Licensed Professional Nurse or Registered
Nurse License in the state the facility resides in Illinois
