Pay Rate

$2,884 to $2,928 per week

Elite Source Health is currently in need of Registered Nurses with Home Health Experience for a 13-week contract for various openings in Chicago. Duties and Responsibilities:

• Demonstrate knowledge of home health regulatory standards and policies of agency.
• Initiates the plan of care and necessary revisions.
• Performs initial evaluation on patient visit and reevaluates the patient's nursing needs on succeeding visits.
• Performs services in accordance with the plan of care.
• Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
  
  Requirements
  
  
  • Minimum of 1 year of current work experience providing in specialty area applying for
  • Current state license in good standing with State License Board
  • HCHB experience highly preferred
