nurse RN Clinic Case Manager, Utilization Review for a local
contract nursing job in Downers Grove, Illinois. Job Description &

Local Contract job in Downers Grove, IL No Travel worker will be
considered. Job Description Position: Nurse Case Manager Location:
Downers Grove, IL (Remote/Hybrid) Duration: 06 Months (with
possibilities of extension) Shift: Mon - Fri 8:30-5:00pm 3 days and
12:30-9pm 2 days CST Note: • Must reside in the state of IL • Valid
and unrestricted IL RN License • Hybrid role - mostly telephonic
(90%) and may need to do member visits if the member requests it
and they are no more than an hour way. Office visits to required
once a quarter. The office is located 3200 Highland, Downers Grove,
IL. If the CW is move than 2 hours away they can do the meeting via
TEAMS. Position Summary • The Care Manager RN is responsible for
driving and supporting care management and care coordination
activities across the continuum of care (assessing, planning,
implementing, coordinating, monitoring, and evaluating). • The CM
RN utilizes advanced clinical judgment and critical thinking skills
to facilitate appropriate member physical and behavioral healthcare
through assessment and care planning, direct provider coordination
/ collaboration, and coordination of psychosocial wrap around
services to promote effective utilization of available resources
and optimal, cost-effective outcomes Duties • Responsible for
telephonic and/or face to face assessment, planning, implementing
and coordinating care management activities with members to ensure
that their medical and behavioral health needs are met and to
enhance the member’s overall wellness. • Through the use of
clinical tools and information/data review, conducts comprehensive
assessments of member’s needs and recommends an approach to case
resolution by meeting needs in alignment with their benefit plan
and available internal and external programs and services.
Experience • 3-5 years clinical practice experience, e.g. hospital
setting or alternative care setting such as ambulatory care or
outpatient clinic/facility • 2 years’ experience using personal
computer, keyboard navigation, navigating multiple systems and
applications; and using MS Office Suite applications (Teams,
Outlook, Word, Excel, etc.) • Case management in an integrated
model preferred • Managed care experience preferred • Discharge
planning experience preferred • Experience providing care to the
Medicaid population preferred • Spanish Speaking Preferred
Education • Unrestricted RN in IL • Minimum Associates or diploma
nursing degree required • Case Management Certification CCM
