Headquartered in Chicago, IL, we currently operate in over 10 states and are rapidly expanding throughout the USA.Please watch our video to better understand how Vitae Health Systems delivers on its mission "to preserve and improve human life" through superior clinical care, academic excellence, and innovative research.VITAE - Nurse Practitioner ProgramThe PositionAs we continue to grow, we are looking for "SNF-ists," in-house nurse practitioners that will provide care for the geriatric population in a Skilled Nursing and/or Assisted Living setting.What You Will DoAs the SNF-ist, you will act as a Skilled Nursing Facility's leading provider of internal medicine services. You will collaborate with the facility's interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all of the facility's residents.You will work with the entire facility's patient population - short and long-term residents - overseeing their day-to-day care with the assistance of the facility's nursing staff and guidance from the facility's medical directors and attending physicians.As a SNF-ist, you will provide types of care, including but not limited to:Be comfortable and proficient with documentation and CPT codingEvaluation of new-admission visitsCoordination of discharge visitsMonthly stability visits for long term residentsEvaluations for patients exhibiting changes in conditionAdvanced Care PlanningOther Duties & ResponsibilitiesProvide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and diseasePrescribe all necessary medications and treatmentsPerform comprehensive physical examinations of patientsDevelop and implement patient management policies and proceduresFacilitate referrals to other healthcare professionals and medical facilitiesCommunicate with attending physicians and/or specialists regarding patient careBe comfortable and proficient with documentation and CPT codingWorking with Vitae will provide you with: - A healthy work-life balance with flexibility.
